Abstract

Despite high EHR adoption, only 70% of hospitals perform all key interoperability functions, affecting pharmacists' ability to access complete medication data. Incomplete interoperability leads to fragmented medication histories, increasing risks during medication reconciliation and transitions of care. Pharmacists face challenges in order verification and clinical decision support due to inadequate data exchange, impacting safety and workflow efficiency. Enhancing EHR interoperability is essential to reducing medication-related harm and improving the accuracy of medication management.
